Consent text
I, the sender of an enquiry on https://sport-health.ru, freely, of my own will and in my own interests, give the International Public Organisation for the Promotion of Science and Sport ‘Sport, People and Health’ (МОО «Спорт, Человек, Здоровье»), OGRN 1117800001274, INN 7814160983, KPP 781401001, registered address: Office 406, 61 Vyborgskaya Embankment, Saint Petersburg, 197342 (the Controller), separate consent to process the personal data I provide.
1. Purposes of processing
- receiving and reviewing an enquiry about partnership, sponsorship or the selected participation format;
- checking its content and forwarding the enquiry to an authorised person;
- contacting me using the details provided, clarifying terms and answering questions;
- recording the content and outcome of business correspondence;
- preventing automated and repeated abuse of the form;
- protecting the parties’ rights and complying with legal requirements.
2. Personal data covered
This consent covers:
- organisation name;
- contact person’s name;
- email address for the reply;
- telephone number, if I provide one;
- enquiry subject and selected partnership package or format;
- selected options and the displayed preliminary estimate;
- the text of my comment;
- submission date and time, delivery status and the minimum technical information necessary to protect the form against abuse and record consent.
I do not include special categories of personal data, health information, passport or bank details, passwords, third-party data or other information unnecessary for answering the enquiry in the free-text comment.
3. Processing operations and methods
The Controller may collect, record, organise, accumulate, store, update, retrieve, use, provide access to authorised persons, block, delete and destroy data using automated means and, where necessary, non-automated means.
4. Parties engaged by the Controller
The Controller may engage providers of Website hosting and technical support, business email and service message delivery to process data. They receive only the data necessary for the relevant service, act on the Controller’s instructions and must maintain confidentiality and security.
Access to the enquiry is granted only to the Controller’s authorised representatives who need it to review the application and respond.
5. Duration and retention
Consent applies from submission of the form until the enquiry and related business correspondence have been concluded, or consent has been withdrawn, whichever occurs first. Data may then be retained for the period necessary to protect the parties’ rights and fulfil mandatory legal requirements, after which it is deleted or destroyed.
6. Withdrawal of consent and data subject rights
I may withdraw consent by sending a request to info@sport-health.ru or to: Office 24, Building A, 8 Malaya Morskaya Street, Saint Petersburg, 191186. The request must state the email address used, my name and my request to withdraw consent. The Controller may request reasonable proof of identity.
Withdrawal does not affect the lawfulness of processing carried out before it was received. Where the Controller has another basis provided for by law, mandatory processing may continue within the scope of that basis.
I may request information about processing and the correction, blocking or destruction of data where lawful grounds exist, and may challenge the Controller’s actions before Roskomnadzor or a court.
7. Consequences of declining consent
Giving consent is voluntary. Without it, the Controller cannot accept and respond to an enquiry through the form. I may write directly to info@sport-health.ru, although the data in the email will also be processed to consider my enquiry.
8. Electronic confirmation
Consent is confirmed by a separate affirmative action: I select the checkbox beside the link to the full text of this Consent and submit the form. The Controller records the Consent version, action date and time, and the minimum information necessary to demonstrate receipt of consent.
This Consent does not authorise publication of the enquiry or my data in the public domain, does not constitute agreement to the terms of a specific partnership contract, and is not consent to advertising or marketing mailings.
